SERVICES

ADULTS:

Adults who are eligible for PRP services must be 18 years or older, have medical assistance, receive mental health services from a physician or licensed mental health provider, and meet criteria for a priority population mental health diagnosis. Adults will be seen at least six times a month face to face. Services are offered in various settings such as Timeless Choice’ facility (onsite); at residence, job, or another appropriate location in the community (offsite); or at a combination of the two (blended onsite and off-site).


Target areas may include, but are not limited to the following:

 Providing psycho-education to assist clients with understanding their diagnoses

 Medication management

 Development of natural supports through increased opportunities for socialization

 Referrals to community resources identified

 Assistance with scheduling and maintaining appointments

 Assistance with securing entitlements (i.e. SSI, SNAP, TCA, local housing agencies)

 Assistance with vital records

 Development of crisis management and wellness for self-care/recovery related skills

 Coordination of care with other providers/treatment team members (i.e. physicians, psychiatrists, therapists, substance abuse providers etc…)

 Life skills training (i.e. grooming, hygiene, nutrition, dietary planning, budgeting/money management, self-administration of medication)

 Mobility Reduced Fare (i.e. applying for reduced cost for transportation)

 Assistance with identifying resources for clients experiencing homelessness

 Crisis Intervention/Safety Planning

 Other areas of need as identified
